New Patient Registration  PreRegistration

The doctors welcome new patients who live within our practice RM3 area. 

To register with this surgery please come in between 13:30 and 15:30 (not on a Wednesday) and bring your NHS number with you.

You are advised to provide:

    • a utility bill

    • a copy of indentification form such as driver licence or passport


As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor if necessary. 

Medical treatment is available from the date of registration.  Please contact reception for further information.
